This award winning service is nurse led and combines home visits with remote monitoring from the comfort of your home or workplace. Results are sent via a phone-line or internet for interpretation and action. Services also include health assessments, mental health reviews or counselling sessions with a nurse, counsellor or psychologist. We can assess from a hospital bed to assist with discharge back home and for your package to start straight away. Our aim is to reduce NHS strains by reducing expenditures, hospital admissions and shortening length of stays.
