SET Family Medical Clinic
www.setofcs.orgSET Family Medical Clinics, is a 501(c)(3) non-profit organization that serves the uninsured & under-insured in the Pikes Peak Region at little or no cost to the patient. SET is helping to ensure that the rising number of uninsured people in the Pikes Peak Region are able to receive the quality health care they need and deserve. SET’s services are available to anyone in the community when unfortunate and unexpected events occur. SET operates a Family Clinic Monday through Friday, including 3 evenings. The Family Clinic hours are M-W 8:00 AM to 8:00 PM and Th-F 8:00 AM to 5:00 PM. The Homeless Clinic at Marian Soup Kitchen operates Monday through Friday from 11:30 to 1:00 PM. The Comprehensive Healthcare Re-entry Program was developed as a community response system for ex-offenders in the Pikes Peak Region. Those who enroll in the program benefit from individualized case management, a peer mentoring program, access to both medical and mental health care that includes prescription medications, and a clearinghouse of information on local programs. The State of Colorado has a 64% recidivism rate for individuals being released from incarceration within the first three years. The CHRP Program now in its third year of operation has seen just a 7% recidivism for our participants. The Senior Well Being clinics are held monthly at 8 HUD subsidized senior housing units, 2 community centers and the Colorado Springs Senior Center. The clinics are staffed by volunteer nurses, pharmacists, dietitians, and social workers. These SET volunteers will review the history and current health of the seniors, providing health and social assessment, education and referrals. We also provide cards to keep track of their blood pressure that is assessed at each visit.
