POST ACUTE CARE PROVIDER IN CONNECTICUT InHouse Care LLC is made up of a group of nurse practitioners that are dedicated to providing on site medical care to residents in post acute rehabilitation, long term care, assisted living facilities and residence. SERVICES PROVIDED BY INHOUSE CARE LLC PRACTITIONERS: 1. Transition of care management 2. Medical care of patients and family members 3. Prevention of avoidable hospital readmission 4. Continuity of care in the nursing home 5. Improved communication and coordination of care InHouse Care APRNS provides clinical health care management in various settings. We are available to serve as providers in the following settings: 1. Nursing homes 2. Long-term care facilities 3. Assisted-living facilities 4. Residential REDUCTION OF AVOIDABLE EMERGENCY ROOM TRANSFERS Collaboration with physician prior to transfer to emergency room. Educate Primary care providers of ability to directly admit to skilled nursing facility within 30 days of discharge without requirement of emergency room visit. We provide medications upon discharge from facility to cover until next primary care appointment. Communication with community primary care providers prior to discharge from nursing home.
