We aim to be your patient centered medical home, providing care from birth through the geriatric (elder) phase of life, with age and gender-specific care available, such as care specific to women's health (annual PAP exams and breast exams), and children's health (annual well-child check-ups and vaccinations). We combine timely preventative care, education, counseling, case management, urgent, and primary care to ensure that our patients and their families have the information and tools necessary to take control of their heath and increase their quality of life. We currently provide referrals for patients in need of specialty healthcare, such as chronic pain or rheumatology. To further show our commitment to help patients with Diabetes and Thyroid conditions, we have an Endocrinologist and a Registered Dietitian serving our patients via telemedicine right here at the clinic.
