South Norfolk Healthcare (SNH) is a Community Interest Company set up by 16 GP practices within South Norfolk. Since 2010 SNH has successfully delivered a range of Referral Information Services, Clinical Education and Analytics to NHS commissioners. SNH strongly believes that referral information services can bring stability to referrals within primary care, keep patient choice and experience at the forefront of decision making, whilst better enabling effective cost management and continuity of services moving into the future.
