ELDAC aged care
www.eldac.com.auEnd of Life Directions for Aged Care (ELDAC) is a project funded by the Australian Government Department of Health to support quality care at the end of life. It is being delivered by a consortium that brings together three universities and five national bodies from aged care, palliative care and primary care. The project seeks to promote changes enabling a reduction of avoidable hospital admissions, with shortened stays, and improved quality of care for people supported in residential and community aged care programs. ELDAC focuses on four streams of work to support these directions: 1. Capacity building through access to toolkits and to online and phone support 2. Improved care and service provision through technology solutions 3. Better understanding of issues through policy briefings and meetings 4. Service and sector development through local and regional partnerships ELDAC is building on existing work and resources to build awareness and encourage their use. It aims to connect those working with older Australians in aged care, primary care and palliative care with the quality resources available and to facilitate awareness of, and connection between, services, agencies and groups. Key resources include the palliative care evidence developed for palliAGED and the expertise of Advance Care Planning Australia. Consortium members: - Queensland University of Technology (QUT) - Flinders University - University of Technology Sydney (UTS) - Palliative Care Australia (PCA) - Aged & Community Services Australia (ACSA) - Leading Age Services Australia (LASA) - Australian Healthcare and Hospitals Association (AHHA) - Catholic Health Australia (CHA)
