OneCity Health is a subsidiary of NYC Health + Hospitals focused on population health, care management, and implementation of the State’s Delivery System Reform Incentive Payment (DSRIP) program. Established in 2014 under the auspices of the DSRIP program, OneCity Health envisions the establishment of a welcoming, accessible, and integrated health delivery system that encourages, supports, strengthens, and protects a state of wellness and healthy living for all. Our transformation efforts include: • Implementing DSRIP initiatives together with NYC Health + Hospitals and over 200 partner organizations representing primary care providers, community-based organizations, behavioral health providers, and SUNY Downstate Medical Center - University Hospital of Brooklyn • Improving access to primary care • Enhancing care management and care coordination for New Yorkers with complex health and social needs • Improved treatment and support for persons with mental health or substance abuse challenges • Integration of health and social services • Delivering culturally competent care • Increasing outreach to patients who have had difficulty connecting with care NYC Health + Hospitals is the largest public health care system in the nation serving more than a million New Yorkers annually in more than 70 patient care locations across the city’s five boroughs.
