"Helping One Community At A Time" by bringing comprehensive diabetes education lead by clinicians to underserved/uninsured communities focusing on diabetes prevention and diabetes self-management to help to improve diabetes outcomes and build stronger communities. Our goal is to decrease the number of diabetes cases and diabetes complications in this patient population. Diabetes "You Can Win" was established to let people know that they can win and have a long healthy life if they have diabetes. We are losing a fight that we can win! By helping families to create healthier life plans we will help them to prevent diabetes and if they have diabetes help them to achieve good management.
