We also support successful transitions from hospital to home, reducing readmission rates and improving well-being, through the provision of telephonic nurse support and clinical services such as HITH, Chemo at Home and REHAB+. Our home care division enables and empowers people to continue living independently in their own home for as long as possible as they age or need to manage chronic health conditions.
