Heath2Works (H2W) has developed Recap Health - a digital patient education platform that enables clinicians to serve their patients better. Why? Because many patients receive very limited and poor information to help them understand their diagnosis and self-care effectively. Now, with Recap Health patients with long-term conditions, patients on waiting lists for surgery, clients with mental health challenges and new parents can all benefit from trusted information and advice delivered by Recap Health. They can increase their health literacy, gain confidence to engage and self-care effectively, and ultimately recover or live better than they otherwise would have.   Recap Health is condition agnostic. It is used by NHS Trusts to support patients across a wide range of services including heart failure, cardiac rehab, COPD, elective surgery (especially MSK), PreAMS, Children and Young Peoples services (especially new parents), adult and older peoples mental health services and public health wellness services. Recap Health has been evaluated as part of the NHS England's Test Bed initiative. It has been proven to reduce readmission rates, increase self-care efficacy scores and improve patient satisfaction scores.
