Heartfelt Technologies exists to help patients, their families, carers and doctors, to reduce hospital re-admission rates from congestive heart failure de-compensation. We solve the problem of consistent data collection for remote patient monitoring. The Heartfelt device routinely collects data on > 16 days out of 30, even for patients with severe adherence difficulties. Regular, consistent data collection is critical to understanding patient condition and good patient management. This is why Medicare will only pay for monitoring of patients where observations are made on at least 16 days out of 30. Unfortunately, this means that doctors and remote patient monitoring (RPM) companies are incentivised to only monitor adherent patients, who are the least likely to benefit from ongoing monitoring - because adherent patients are the most likely to proactively engage with healthcare. The Heartfelt device is passive in nature, allowing it to capture data from patients practically every day, unlike all other non-invasive telemonitoring systems. This is a revolutionary solution for remote patient monitoring, doctors and RPM companies to provide better patient care and bill more regularly for even their most challenging patients. In the USA, heart failure costs Medicare over 30 billion per year and one in five people will suffer from it. It is the most common cause of hospitalisation for those aged over 65, and half of those patients will have repeat hospitalisations. Yet, it is estimated that three-quarters of these repeat emergency admissions could be avoided. The key problem to address is that non-adherence is strongly associated with hospitalisation risk, and that very few options are available to monitor non-adherent patients.
