achi is a holistic care management company that lowers your expenses by actively engaging people and connecting them to organizations to address the social determinants of health. achi was born out of a desire to improve our community and to transform our healthcare payment system into one that is sustainable in the future and aligns desired behaviors with payments. Our focus is on working with health systems/insurance companies, educational institutions, and large businesses - anyone that is at risk for a certain population. By actively communicating with the desired population and connecting them to resources to help address their social determinants of health, we can increase engagement/retention and decrease health care and turnover expenses. By showing that investment in the right programs upfront will both positively change lives and reduce expenses, we also want to transform our healthcare payment systems. Our healthcare payment model is not sustainable as is, so aligning payments to value and outcomes will be required. Collaboration and cross-sector partnerships will be required to truly move the needle in addressing social issues in our communities.
