We are an online resource hub dedicated to helping Ontario youth navigate the adult disability system. We help in the transition process from youth to adulthood by: (1) Providing information on components of the disability system, including finances, healthcare, recreation and housing services. (2) Sharing personalized community resources to connect youth to the adult disability system. (3) Facilitating conversations between youth, families, and care providers to support a successful transition to adulthood.
