The Agency, which was founded in 1971, is made up of a team-oriented group of specially trained professionals, who work together with patients and family members to develop comprehensive care plans for either Home Health or Hospice Services, and/or Support Services; specific to the needs of the patient. Trained professionals include physicians, nurses, social workers, aides, therapists, and specially-trained hospice volunteers. The Agency’s Visiting Nurses travel the vast service area 7 days per week, 365 days per year. North Country Home Health & Hospice Agency provides home care and hospice services to a 48-town services area throughout Coos County and Northern Grafton County. Services are provided to all ages, from infant to elderly. Payment sources include Medicare, New Hampshire Medicaid, private insurance and private pay with a sliding fee scale. NCHHHA is a part of North Country Healthcare, which has a total of five affiliates in the North Country (Androscoggin Valley Hospital, Littleton Regional Healthcare, Upper Connecticut Valley Hospital, Weeks Medical Center and North Country Home Health & Hospice Agency.
