PCHP leverages the national network of home & community based organizations for the assessment in social determinates of health(SDOH) to identify gaps in care and coordinates the closure in gaps that drive up the cost and utilization of health care. Through a single contract PCHP can bring scale of a national community-based provider network that is proven to drive down the cost of health care by navigating the SDOH.
