The Patient Care Intervention Center (PCIC) is a non-profit organization that works with patients and healthcare providers to improve quality of care and reduce cost for the most complex and costly patients in the system –“High-Need, High-Cost” (HNHC) patients. HNHC complex patients are those that tend to use Emergency transportation or visit the Emergency Room 10 times or more per year and are usually suffering from multiple chronic medical and behavioral conditions. PCIC is developing a new model of care that focuses on creating goal oriented care plans with the patients. This model also focuses on creating new ways to resolve barriers at the individual and systemic levels, and supporting technology solutions that encourages hospitals, health systems, FQHC’s, managed care organizations, social resource and government agencies to share and integrate data, improve communication, and coordinate care plans across agencies (while reducing costs).
