We provide our patient navigation services to pediatric and adult congenital heart disease(CHD) patients free of charge. We give them the tools to navigate CHD by helping them understand their condition, preparing them for their appointments and procedures, empowering them to advocate for themselves, and assisting them in getting the resources available to them to support them throughout their journey.
