What We Do: MassCare proactively works with chronically ill patients to change their lifestyle and behaviors to improve outcomes and therefore lower the overall cost of healthcare. At the same time, our program increases the level of reimbursements physicians and hospital systems earn from Medicare and commercial insurance companies. How Do We Do It?: We place Care Liaisons including Clinical Pharmacists into physician practices to manage their chronically ill patients, providing them with medication reviews, Annual Wellness Visits and monthly Chronic Care Management counseling.
