PCC Community Wellness Center is a network of 14 health centers serving Chicago’s West and Northwest Sides and the near west suburbs. PCC operates as a medical home, where we offer comprehensive primary care with the integration of behavioral health and oral health care. Services are offered conveniently within one network, and in some cases, one facility. By providing these services in a coordinated fashion, patients benefit from a high level of care continuity while facing fewer barriers to care. These additional support services are not an afterthought, but rather essential elements of accomplishing good health. The mission of PCC is to improve health outcomes for the medically underserved community through the provision of high quality, affordable, and accessible primary health care and support services. Anchored with family medicine, we are committed to serving the needs of all people in all stages of life.
