We are a Health Home Program providing care coordination services to adults and children on Patient 1st Medicaid. To qualify for our services you must have at least one of the following health problems: • Diabetes • Asthma • COPD • Heart Disease • Organ Transplant Needs • Substance Abuse Disorders • Sickle Cell Anemia • Cancer • Mental Health Conditions • HIV • BMI over 25 • Hepatitis C
