Formed in 2006, CLH is Central London’s GP Federation. CLH is a community interest company, a not-for-profit organisation which is owned and run by local primary care clinicians to improve health and wellbeing in Central London. CLH has an elected non- Executive Board of Directors which is made up of GPs, Practice Managers and Practice Nurses from member practices. CLH is the single GP provider network and delivers healthcare services to over 200,000 people. These services include a primary care mental health service called Primary Care Plus (PCP), a Care Coordination Service (CCS), 19 Out-of-Hospital Services, and we are a provider partner in the Community Independent Service (CIS), an integrated health and social care service aimed at helping avoid hospital admissions and reducing dependency on long-term services.
