Alliance Health Centers' goal is to provide education and help manage your health at all stages of your life by providing access to high quality primary and behavioral health services. Translation services are available as needed. Also, a certified insurance navigator is on-site at Alliance Health Centers to assist individuals in determining eligibility and enrolling in health insurance coverage such as Medicaid. We are conveniently located in southeast Fort Wayne at the corner of Lafayette and Pontiac within the Lafayette Medical Center. We are accepting new patients and welcome walk-ins.
