We are currently in the Pilot stage and are seeking clinics that have implemented Collaborative Care and are looking for better tooling to help improve their efficiencies. At Akello we are building tools to help enable collaborative care teams to support mental health populations in the Primary Care setting. We currently focus on the CoCM model. Our first solution is a digital patient registry designed to improve engagement and collaboration for CoCM teams and help them follow protocols correctly.
