UCHAPS is a national collaboration of community partners and health departments with a history of dedication to preventing new HIV infections and reducing health disparities, morbidity, and mortality related to HIV and AIDS. As a coalition of local community members & health departments from jurisdictions heavily impacted by HIV/AIDS (Atlanta, Broward County, Houston, Baltimore, NY, Philadelphia, San Francisco and DC), we focus our attention on addressing HIV, the social determinants of health and achieving health equity through a platform of stakeholder engagement, peer learning, mobilization and coalition building. UCHAPS employs a structure of communication that facilitates engagement, mobilization, information sharing and delivery of peer-to-peer technical assistance for health departments and community prevention partners. UCHAPS members meet in-person at least three times a year and utilize a Peer-Based Technical Assistance Model to identify/meet technical assistance needs; share lessons learned; develop strategies to reduce redundancy; and, eliminate costly approaches that have been implemented and not achieved the intended impacts. UCHAPS produces guidance/best practice documents from its regional/national meetings based on the experiences of gay/bisexual men of color; women of color, including those of transgender experiences; those residing in urban and rural environments; people who use drugs; and, those impacted by poverty & displacement. The goal of this work is to inspire social, civic and political actions that maximize resources, inform community and science-based strategies, and produce cost-effective mechanism to share best practices and motivate actions.
