HomeLink Healthcare is a clinically-led organisation providing safe, high-quality Hospital at Home services to NHS patients. By working in partnership with hospitals, consultants, community providers and GPs to provide ‘out of hospital’ care we can prevent some people being admitted to hospital and enable others to come home more quickly. Our highly skilled multi-disciplinary nursing and therapeutic teams can safely provide holistic care in the patient’s home, seven days a week. We use remote patient monitoring technologies. These provide effective monitoring and data-driven learning for the well-being of patients and effectiveness of the health system. Our services: • free up hospital ward capacity • improve patient flow • offer patients more choice. We work through four clinical pathways delivered by nurses, therapists and support staff: • virtual wards • admission avoidance • early supported discharge • reablement The features of our work across these pathways include: • working in partnership with other providers • pre-discharge assessment • facilitating same day transfer/discharge • home based care up to QDS (four times a day), seven days a week • a 24/7 on call service, every day of the year • Real time activity data / analytics • KPI’s and business intelligence reporting.
