CHC is a Joint Commission accredited home care company that services patients across the state of Massachusetts. Our vision supports giving exceptional care to patients while being mindful of not creating dependence on services, but to teach independence and self-care in a way that allows the patient to live and remain safe in our community. We are a skilled in-home nursing agency that offers nursing, therapies and home health aide to help individuals live independently in the community. Eighty percent of CHC’s patients have mental health and addiction diagnoses, as well as standard medical diagnoses. We are the only home care company in the state who prepares all of its qualified nurses to sit for the examination to become board certified psychiatric nurses. With that being said, we work very closely with psychiatric clinics and hospitals and are supported by NAMI in caring for this very fragile population in our state. CHC has had a utilization review team in place for years for the purpose of assessing visit frequency and looking at outcome measures to assure that we are using the LEAST frequent visit numbers to support the patient in the community and assist in keeping them out of hospitals and emergency departments. We opened in 2007 and currently have offices in Methuen, Worcester, Leominster, Peabody Springfield and Millbury Massachusetts.
