The Upper Grand Family Health Team provides a broad range of health-care services that focus on health promotion, disease prevention and chronic disease management. Our clinical team consists of nurse specialists, dietitians, mental health therapists, nurse practitioners, health promoter, pharmacist, occupational therapist and physician assistant who are supported by administrative staff to serve the residents of Centre Wellington. We work closely with our affiliated physicians to ensure that patients are well-supported to achieve their healthcare goals.
