House Case Management Services -Purpose: Individuals and families who receive Case Management Services access and use re-sources and supports that build on their strengths and meet their service needs. Outpatient Mental Health Services. Purpose: Individuals who receive Outpatient Mental Health Services that target goal-directed interventions for diagnosable conditions make gains in symptom reduction, improved self-management, and restored or enhanced daily functioning.
