Founded in 1999, Delmont Medical Care is a multi-site, multispecialty practice comprised of twelve Primary Care Physicians (PCP)’s and more than eighty (80) employees. The practice is recognized as a Level 3 Patient Centered Medical Home (PCMH) under National Committee on Quality Assurance (NCQA) standards. Delmont Medical Care delivers efficient, high quality care to thousands of low-income, uninsured and under-insured patients across Long Island and Queens. Patients are seen in practices in Freeport, Franklin Square and Far Rockaway, while a dedicated call center is located in Westbury.
