Our aim is to work collaboratively with referring partners and their Clients. We are passionate about providing connection and meaningful support. Our team have the skillset which enables us to provide meaningful support work and participate within a Care Team to ensure continuity of care, as may be desired. We believe you and your clients will be satisfied with our Mental Health Workers due to our level of client engagement and unique method to facilitate meaningful connection between client and worker. We enjoy providing psychosocial support services in the following areas:  High Intensity Daily Personal Activities  Assistance to with Social, Economic and Community Participation  Psychosocial recovery coaching  Transport We operate in the following catchments: Inner East Melbourne, Bayside, Outer East, South East We provide supports to Clients who’s plans are; Plan Managed and Self-Managed. Our qualified Mental Health Workers have the following qualifications (As a minimum):  Certificate IV in Mental health or equivalent  First Aid & CPR Certificate  Police check  Working with Children check  Disability worker exclusion scheme check  NDIS worker orientation module “Quality, Safety & You” What you can expect from us:  No long referral forms! An Initial discussion with you to review your needs  We will phone the Client to better understand their needs  Face to face meeting with Client at their home  Unique process to match two of our qualified Mental Health Workers to the client’s personality, plan and needs.  Regular Client progress updates We believe we can be the ones that help our Clients to live their best life!
