Talk Health with Frankie Co., Inc, a 501©3 non-profit holistic health practice, and Durham Family Medicine, a primary care practice located in Durham, NC are teaming up to establish a partnership/community-based collaboration that will bring Durham residents health coaching. The collaboration was created in 2015 when the growing need for health coaches became necessary for engaging and incorporating patients in their own health care. Talk Health with Frankie Co, Inc was established to address the needs of patients at Durham Family Medicine (DFM) and the surrounding community. Those needs include access to healthy eating options such as community gardens, safe walking trails, group fitness sessions, healthy stress relieving activities such as meditation, and family activities that encourage fitnessand wellness. Our goal is to provide compassionate care to all patients especially those at higher risk for chronic diseases. Our health professionals are dedicated to helping people realize their full potential regarding their health, lifestyle and attitude. We want to help every patient become, and remain, healthy and strong. We do this by listening to patients needs and providing them resources that help them achieve optimal health.
