Long Term Care & Rehab Consultants, LLC was founded in 2008 by Dr. Reza Esfahani to help raise the bar in transition of care - the movement a patient makes between health care practitioners and settings as their condition and care needs change. Our team, comprised of highly qualified board certified physicians, physician assistants and nurse practitioners, works in concert with the multi-disciplinary team at the facility to direct clinical care.
