Choice Matters provides a full range of Skilled Medical Services in the greater Jacksonville and surrounding areas, all in the comfort of the patient's home. Our Nurses, Therapists and Medical Social Workers are qualified to provide all levels of clinical treatment and clinical case management services. We work closely with physicians, hospitals, rehabilitation facilities, skilled nursing facilities and families; forming a circle of support around our patients. This approach results in better communication across treating providers and improved outcomes for the patient. Choice Matters also offers non-skilled services, provided by licensed Certified Nursing Assistants, for patients who have extended stays in hospitals or rehabilitation facilities and who may require extra attention.
