Senior Care Connections is an independent, professional care management practice that provides assessment, planning and coordination of services to meet the needs of older adults and their families. Our clients are seniors or disabled adults who need assistance managing the details of their daily lives. We assist family members or others who are responsible for the welfare of frail adults whether they are living in their own homes, assisted living communities or nursing homes.
