www.projectaccessnow.orgProject Access NOW (PANOW) works to increase access to health care, makes that care more equitable, and comprehensively addresses factors that negatively impact the social determinants of health. Each year, the organization serves 20,000+ individuals across Oregon and SW Washington and has formal agreements with most major health care systems regionally. PANOW is a bridge helping to facilitate care for the most vulnerable individuals. • Classic Care Coordination works with hospitals and clinics to facilitate donated medical care. • Pharmacy Bridge facilitates access to low cost medications for those without health insurance. • Outreach, Enrollment, and Access helps individuals enroll in health insurance and does outreach to distribute information about health resources, testing, vaccines, and changing health and safety guidelines. • Premium Assistance aids those who earn too much to qualify for Medicaid but too little to afford private insurance, paying premiums and reducing out-of-pocket expenses. • The C3 Community Assistance Program supports discharge planners and social workers, connecting low-income patients to the resources (i.e. emergency housing) needed for a safe hospital discharge and a return to wellness.
