Connections Case Management, LLC is committed to providing appropriate services for children and adults with intellectual and/or developmental disabilities. We assist individuals and their families in connecting with therapies, services, and resources so that they can remain in their homes and communities. This is accomplished through a person-centered approach that identifies the individual’s strengths and support needs in order to develop an effective plan for services.
