Health Connections aims to have a positive influence on the health and happiness of others through connecting and strengthening the community and unearthing hidden assets. We support people to actively participate in managing their own and others (if they are Carers), health, care and wellbeing in ways that matter to them. We value a partnership approach by recognising and promoting the value of community, public, private and third sector collaborations and co-production to build healthy, supportive communities.
