Strong Children Wellness (SCW) is a network of advanced pediatric primary care practices that fully integrates primary care and telehealth services into community-based social service organizations in New York City. By embedding primary care hubs into community-based organizations that offer comprehensive mental health, social, and care management services, SCW’s innovative ‘reverse integration’ model allows us to provide families with a personalized, multidisciplinary care team that addresses physical, developmental, emotional and social needs holistically.
