Comprehensive Care Connections (C3) is a group of Community-Based Organizations providing service coordination for older adults and those with disabilities who want to live safe, healthy, and happy at home. Whether they are healthy and active or needing higher levels of care, the Community-Based Organizations in C3 know the resources older adults need to remain happy and healthy in the environment of their choosing.
