The IPH Center brings together trans-disciplinary experts with the goal of improving population health, well-being, and equity. To achieve a “culture of health,” the IPH Center emphasizes four strategic goals: • Make health a shared value. • Foster cross-sector collaboration to improve well-being. • Create healthier and more equitable communities. • Strengthen the integration of health services and systems. We address the population health challenges through a Transformational Collaborative Outcomes Management (TCOM) Framework.
