**This page is discontinued.** The Erie St. Clair Community Care Access Centre (CCAC) connects you with the care you need, at home and in your community. Funded by the Ministry of Health and Long-Term Care, through the Erie St. Clair Local Health Integration Network (LHIN), we help you navigate through a complex health care system. By connecting you to appropriate and available health care and social services in your community, we act as a one-stop access point to community care. In addition, we help patients maintain independence in their own homes with dignity, arrange in-home health care services, and support patients through the application and admission process to long-term care homes when living at home is no longer possible.
