Affinia Health Network is a Clinically Integrated Network (CIN), bringing together a health network of over 1,400 physicians, four hospitals, and other professional health care professionals, working together, using proven protocols and measures, to improve patient care, decrease health care costs, and raise value. We will transform health care in West Michigan through effective collaborative relationships with physicians, health systems, payers, patients and communities. Care will be delivered in patient centered settings, assuring timely access with a focus on quality, service and cost.
