Piedmont Health Partners is a clinically integrated network of physicians, providers and facilities coordinating and managing services for patient populations across the continuum of care. Piedmont Health Partners was founded by AnMed Health in partnership with the medical community with the aim of: Controlling the Costs of Health Care Improving the Health of our Community Enhancing the Patient Experience Improving the Work Life of Clinicians and Care Givers On one hand, our Health Partners are the providers and facilities that make up our network. On the other hand, Piedmont Health Partners is comprised of nurses, paramedics, social workers, respiratory care practitioners, certified medical assistants and other healthcare professionals we call “Health Partners". These health partners facilitate an enhanced relationship with patients and providers in and out of the hospital.
