Community Health Network is a Federally Qualified Health Center (FQHC) in Brazoria County, Texas that provides professional growth and development in a group practice providing primary health care for adults and children, women’s health services, behavioral health and general dentistry. Stephen F. Austin Community Health Network is dedicated to continuous quality improvement and the ideal candidate will work within a team to insure comprehensive and excellent care. SFACHN clinics are dedicated to Patient Centered Medical Home (PCMH) principles and practices. In 2015, Community Health Network elected to develop an open source electronic health record and modifying it to support the needs of an FQHC. Community Health Network partnered with Williams Medical Technology in this endeavor which required significant additions to the system; including, design, documentation, reporting, workflow, and billing. Community Health Network key staff members and Williams Medical Technology programming staff meet at least weekly to further the development of interoperability, clinic workflow, decision support systems, data warehouse and patient communications.
