Post-Acute Care Resources is a consulting service that assists providers best position their post-acute services including inpatient rehabilitation, skilled nursing, home health, outpatient rehabilitation and hospice. Our approach is to be a resource that offers extensive knowledge and years of experience in post-acute care to our clients to identify the opportunities for improvement and then, in collaboration with our clients, develop action plans to remedy the issues.
