Patients and their loved ones have heightened vulnerability when receiving a life-altering diagnosis, but are suddenly required to learn large amounts of complex information. The shortage of personnel to provide individualized teaching, overwhelming handouts & limited health literacy act as learning barriers, which ultimately results in frustrated & poorly informed patients. Our mobile health applications are designed as a health coach, or personal RN, that is present upon admission and stays throughout the continuum of care providing personalized information for patients and their loved ones. Enhanced inpatient education that is customized to the health literacy levels of patients and caregivers engages them to “become fluent in the language of their disease”, resulting in more compliance and better outcomes.
