Established in 2019 - Health Chain is creating a data-driven healthcare community by digitally connecting Patients, Payers and Providers on an open network. Our platform is the single access point to manage complex, data-driven processes with intuitive technologies in order to put patients at the center of the healthcare ecosystem. The healthcare industry has struggled to move at the same pace as others when it comes to digital transformation, and that is largely due to the lack of accessible, actionable data. Health Chain helps Payers integrate patient data from across their environment (claims, providers, other payers, EMRs, etc.), and we have built solutions for the obstacles that payers currently face when trying to leverage this data - resulting in an interoperable, data-driven health plan.
