A care network is an organization of physicians and other health professionals that provides consistent support to patients in order to improve their overall health and better use of resources. To this end, networks promote access to care, coordination, continuity and interdisciplinarity of care. They encourage the sharing and use of the knowledge of the various actors in the network, in particular through continuous quality improvement initiatives.
