Our free app provides an easy way for friends and family to form a care community, offer help, and get peer support. Our client, the home care provider, obtains patient need and community engagement data based on app usage. This will lead to optimized resource deployment, improved care outcomes, fewer hospitalizations, and greater patient retention rates. As our app engages the community, the elderly feel less lonely and have more agency. We are launching our application with a large home-based health services provider in 2022.
