Bridges is a Supports Coordination Agency in the state of NJ focusing in Hunterdon, Warren, Somerset and Morris Counties. Here at Bridges, we are dedicated to assisting you in discovering services and supports that maximize the potential of those you care about. We will work with you on writing your NJISP as well as coordinate and monitor your plan and services. Once your plan is developed and approved by DDD, Bridges support coordinators will continue to support you throughout your journey. This will be done with frequent contact- at least monthly- either by phone or face-to-face visits. A home visit will be conducted yearly.
